The Coach agrees to provide the client with, and the Client agrees to pay for personal coaching sessions that will be conducted on a weekly/fortnightly basis (unless otherwise agreed) by online messenger/Zoom/Google meets or as mutually agreed prior to each session. All communication about booking coaching sessions and clients intention setting for the session is to be done through email/google calendar/ form sent by the coach. The coach does not accept communication by voice messages.
The Coach agrees to conduct herself in terms of coaching code of ethics, commits to act fairly and in the best interest of the client at all times during coaching.
The Client agrees that his/her primary motive for entering into this coaching agreement is to create and sustain positive change in his/her life. For this purpose the Client undertakes to cooperate with the Coach in establishing and promoting and effective coaching relationship by, amongst other things, listening carefully to the relevant information, tools and techniques provided to him/her by the Coach, communicating openly with the Coach and making genuine and sincere efforts to meet his/her goals as set and agreed to during the coaching sessions.
The client acknowledges that all coaching services provided under this agreement are undertaken completely at the Client's own risk. The Client understands that the coaching process may involve the Coach working with the Client to examine aspects of the Client's life, entering into detailed discussion with the Client about the Client's values and beliefs, making empowering requests of the Client and identifying and agreeing appropriate plans of action for the Client. The Client further acknowledges that, for coaching to be effective, the Coach may decide to ask the Client direct and personal questions and/or address issues in the sessions that could cause the Client to experience strong emotions.
The Client acknowledges and agrees that the Coach does not guarantee results or provide warranties in respect of the coaching services and that the Coach shall not be responsible to the Client or to any third party for any physical or mental condition or circumstance caused as the direct or indirect result of, or related to, the coaching services in any way, including the Client's failure to achieve agreed goals. The Client agrees to indemnify the Coach in relation to any claim(s) made against them for loss or damage suffered as a result of the Client's acts and omissions.
The Client acknowledges and agrees that the process of coaching does not constitute professional consultation, advice, councelling, therapy or medical treatment in any form. In the event that the Coach feels it appropriate for the Client to seek alternative professional assistance, the Coach shall give suggestions. In such cases the Client agrees to abide by the Coach's decision and give due consideration to the Coach's suggestions with regard to referral.
Each coaching session will run for a minimum of one hour, unless ended earlier by the Client. Unless otherwise agreed with the Coach, the Client with pay for all the sessions at the agreed rate. Payment is due at the commencement or before each session. If the Client delays payment without the Coach's agreement, the Coach will be entitled to refuse to provide further coaching sessions to the Client. If the Client does not attend the session and provides less than 24 hours notice to the Coach to this effect, the Coach is entitled to charge the client in full for that session.
The Coach is entitled to terminate the coaching agreement if the client fails to pay the Client's coaching fees, otherwise breaches this agreement, has any insolvency proceedings brought against him/her, engages in harassment, abuse or discrimination against the Coach during sessions, is convicted of an offence or, in the Coach's reasonable opinion, will not benefit from coaching.
Confidentiality - The parties agree to respect the confidentiality of any information provided to each other during the coaching sessions and not to disclose such to any third party without the other's consent. Please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to the protection of any legally recognized privilege. The Coach agrees not to disclose any information pertaining to the Client without the Client's consent. The Coach will not disclose the Client's name as a reference without the Client's consent. Confidential Information does not include information that: (a) was in the Coach's possession prior to its being furnished by the Client; (b) is generally known to the public or in the Client's industry; (c) is obtained by the Coach from a third party, without breach of any obligation to the Client; (d) is independently developed by the Coach without use of or reference to the Client's confidential information; or (e) the Coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) is disclosed to the Coach and as a result of such disclosure the Coach reasonably believes there to be an imminent or likely risk of danger or harm to the Client or others; and (g) involves illegal activity. The Client also acknowledges his or her continuing obligation to raise any confidentiality questions or concerns with the Coach in a timely manner.

Коучинг/психосоматичне цілення - особисті сесії
Коуч погоджується надавати клієнту, а Клієнт погоджується оплачувати персональні коучингові сесії, які проводитимуться щотижня/раз на два тижні (якщо немає інакших домовленостей) відео дзвінком у Zoom/Google Meets або за взаємною домовленістю перед кожною сесією. Вся комунікація щодо бронювання коучингових сесій та встановлення намірів клієнта щодо сесії має здійснюватися через електронну пошту/календар Google/форму, надіслану коучем. Коуч не приймає голосові повідомлення.
Коуч погоджується діяти відповідно до кодексу коучингової етики, зобов'язується діяти чесно та в найкращих інтересах клієнта протягом усього часу коучингу.
Клієнт погоджується з тим, що його/її основним мотивом для укладення цієї коучингової угоди є створення та підтримка позитивних змін у його/її житті. З цією метою Клієнт зобов'язується співпрацювати з Коучем у встановленні та просуванні ефективних коучингових стосунків, серед іншого, уважно слухаючи відповідну інформацію, інструменти та методики, надані йому/їй Коучем, відкрито спілкуючись з Коучем та докладаючи щирих та щирих зусиль для досягнення своїх цілей, встановлених та узгоджених під час коучингових сесій.
Клієнт визнає, що всі коучингові послуги, що надаються за цією угодою, здійснюються повністю на власний ризик Клієнта. Клієнт розуміє, що процес коучингу може включати роботу Коуча з Клієнтом для вивчення аспектів життя Клієнта, детальне обговорення з Клієнтом цінностей та переконань Клієнта, висування Клієнту запитів щодо розширення можливостей, а також визначення та узгодження відповідних планів дій для Клієнта. Клієнт також визнає, що для ефективності коучингу Коуч може ставити Клієнту прямі та особисті запитання та/або розглянути на сесіях проблеми, які можуть викликати у Клієнта сильні емоції.
Клієнт визнає та погоджується з тим, що Коуч не гарантує результатів та не надає гарантій щодо коучингових послуг, і що Коуч не несе відповідальності перед Клієнтом або будь-якою третьою стороною за будь-який фізичний чи психічний стан чи обставину, спричинені прямим чи непрямим результатом коучингових послуг або пов'язані з ними будь-яким чином, включаючи нездатність Клієнта досягти узгоджених цілей. Клієнт погоджується відшкодувати Коучу збитки щодо будь-яких претензій, пред'явлених проти нього, за втрати або шкоду, понесені в результаті дій та бездіяльності Клієнта.
Клієнт визнає та погоджується з тим, що процес коучингу не є професійною консультацією, порадою, консультуванням, терапією чи медичним лікуванням у будь-якій формі. У випадку, якщо Коуч вважає за доцільне для Клієнта звернутися за альтернативною професійною допомогою, надасьб пропозиції. У таких випадках Клієнт погоджується взяти до уваги пропозицію Коуча щодо направлення.
Кожна коуч-сесія триватиме одну годину, якщо Клієнт не завершить її раніше. Якщо інше не домовлено з Коучем, Клієнт оплачує всі сесії за узгодженою ставкою. Оплата здійснюється на початку або перед кожною сесією. Якщо Клієнт затримує оплату без згоди Коуча, Коуч має право відмовити Клієнту в проведенні подальших коуч-сесій. Якщо Клієнт не відвідує сесію та повідомляє про це Коуча менш ніж за 24 години, Коуч має право стягнути з клієнта повну плату за цю сесію.
Коуч має право розірвати коуч-договір, якщо клієнт не сплачує коуч-гонорари Клієнта, іншим чином порушує цю угоду, проти нього/неї порушено будь-яке провадження про неплатоспроможність, він/вона вчиняє домагання, зловживання або дискримінацію щодо Коуча під час сесій, його/її засуджено за правопорушення або, на обґрунтовану думку Коуча, він/вона не отримає користі від коучингу.
Конфіденційність - Сторони погоджуються поважати конфіденційність будь-якої інформації, наданої одна одній під час коучингових сесій, та не розголошувати її третім сторонам без згоди іншої сторони. Стосунки між Коучем і Клієнтом не вважаються юридично конфіденційними стосунками (як у медичній та юридичній професії), і тому спілкування не підлягає захисту жодних юридично визнаних привілеїв. Коуч погоджується не розголошувати жодної інформації, що стосується Клієнта, без його згоди. Коуч не розголошуватиме ім'я Клієнта без його згоди. Конфіденційна інформація не включає інформацію, яка: (a) була у володінні Коуча до її надання Клієнтом; (b) є загальновідомою громадськості або в галузі Клієнта; (c) отримана Коучем від третьої сторони без порушення будь-яких зобов'язань перед Клієнтом; (d) самостійно розроблена Коучем без використання або посилання на конфіденційну інформацію Клієнта; або (e) Коуч зобов'язаний розкрити її згідно із законом, законно виданою повісткою до суду або рішенням суду; (f) розкрита Коучу, і в результаті такого розкриття Коуч обґрунтовано вважає, що існує неминучий або ймовірний ризик небезпеки чи шкоди для Клієнта або інших осіб; та (g) пов'язана з незаконною діяльністю. Клієнт також визнає своє постійне зобов'язання своєчасно порушувати будь-які питання або занепокоєння щодо конфіденційності з Коучем.
